By checking this box I agree that I am a current policyholder of the above insurance company, Capitol Benefits, LLC, is my agent, and my policy is active. I also understand that this is a temporary ID card and that my actual policy will not be changed until the next business day. The coverage will be retroactive to the date submitted. I will receive a confirmation email once the policy has been changed. I understand that if I do not get that confirmation email, that I need to call Capitol Benefits, LLC, at 301-431-0000 within one additional business day to make the change. I agree that all information has been submitted accurately. Further, I (the person submitting this request) am the policyholder of record.
